New Patients Forms
Please print following forms and bring them to your first visit.
We will:
- Work diligently together to find the root cause of your illness
- We will do lab work with a naturopathic review, other naturopathic testing, and lots of talking to get history and timelines
- Construct a doable plan (Together) that is customized for you to remove the obstacles to cure
- This will consist of diet, supplements, and/ or therapies
- Give you unlimited support via text messaging, emails, in person visits and loads of resources (blogs, newsletter, books and videos)
The Process for becoming a Vital One:
- Fill out the Patient questionnaire

- Gather all lab work that you have had done in the last 3 months (ONLY)

- Choose an open time for a new patient phone consult
- Choose a time for In-Office measurements
